Legal Assistance for Seniors /Health Insurance Counseling and Advocacy Program (LAS/HICAP) staff will explain Advance Health Care Directives and how to complete them. They will give information about appointing an agent, end-of-life decision-making and POLST forms. Referrals will be provided for other planning tools, such as wills and Durable Powers of Attorney for Finance.
